• Conduct UAS assessments via rremote telehealth platforms
• Sign and finalize UAS assessments in Health Commerce System within 24 hours of visit
• Submit any required documentation to agency within 24 hours of visit

• Current and Valid NYS RN License
• Current and valid UAS-NY certification for role LTC-40
• Must carry Medical Professional Liability coverage
• Intermediate ability to use computerized systems such as Gmail, Microsoft Word, Excel, and to navigate the DOH UAS health care system

• Bilingual Spanish/English
• Medicaid, Medicare, Managed Care experience
• Experience in Home & Community based or Long Term Care services delivery
• Experience working with the needs of vulnerable populations and passionate about patient care
• Excellent written communication/documentation skills
