Home Health Physical Therapist
 

Responsible for the assessment, delivery, supervision and evaluation of physical therapy services to agency
patients in accordance with the established physicians’ Plan of Care and in compliance with the applicable
federal, state and agency regulations, policies, and procedures.

Education Requirements
• Bachelors of Science degree in Physical Therapy
• Current PT license in the state(s) of practice
 
Experience Requirements
  • Minimum of one (1) year clinical experience, preferably in an acute care or rehabilitation setting
  • At least 18 years of age
  • Reliable transportation
  • driver's license
  • proof of current auto insurance
