Home Health  Licensed Occupational Therapist
 
Responsible for the assessment, delivery, supervision and evaluation of occupational therapy services to

agency patients in accordance with the established physician’s Plan of Care and in compliance with the
applicable federal, state and agency regulations, policies, and procedures.

This is a float position.

Education Requirements
• Bachelors of Science degree in Occupational Therapy 
• Current license in the state(s) of practice
 
Experience Requirements
  •  Six (6) months experience as a practical/vocational nurse in a hospital, nursing home or home care setting within the last three (3) years preferred.
  • At least 18 years of age
  • 6 months experience caring for the elderly or disabled
  • Reliable transportation
  • driver's license
  • proof of current auto insurance
