The Nurse Practitioner Innovations Telehealth Clinician provides virtual support to telehealth patients and families/caregivers in combination with community-based homecare. The Innovations Telehealth Clinician demonstrates technical competency in the use of computer and telephone systems, computer applications, maintaining accurate patient medical records and scheduling virtual phone appointments. In addition, they will assess and support the delivery of services consistent with availability of resources to meet daily needs. The Nurse Practitioner will also use his/her experience and knowledge to provide medication management while supporting the goals of care and advanced care planning needs. This also includes identifying advanced illness and end of life signs.
Location: Washington, D.C.
Schedule: Monday-Friday full-time days

Working in collaboration with other members of the patient’s care management team, the Innovations Telehealth Clinician utilizes effective communication and customer service skills to provide essential care coordination. Having a compassionate presence and lending support, they will identify the need for early intervention related to symptom management or psychosocial needs.
Must have five years of clinical practice experience. Experience in hospice, home health, community health, private duty, and/or outpatient services is preferred. A minimum of one-year experience with telehealth systems and service programs is preferred.
Must have Nurse Practitioner credentials from an accredited program.
Required Certificates and/or Licenses
Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is also acceptable. A certification in Case Management is strongly desired, either at hire or within 12 months of employment following the requirements as outlined by the CCM certification standards.
Must have a driver's license in good standing and access to a vehicle during working hours.
